Job Description
Join UnitedHealthcare as a Field Based Care Coordinator, where you will manage complex health needs for older adult members. Your role will focus on providing personalized care and support to improve their quality of life while collaborating with healthcare teams and community resources.
Key Responsibilities
Serve as primary care manager for high medical risk members with comorbid behavioral health needs
Engage members face-to-face and/or telephonically to complete comprehensive needs assessments
Develop and implement individualized, person-centered care plans
Partner and collaborate with internal care team, providers, and community resources
Provide referral and linkage as appropriate and accepted by the member
Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse in Indiana
Master’s Degree in Social Work
Bachelor’s Degree in Social Work, Psychology, Special Education, Counseling, or relevant degree with at least 1+ years of case management experience
Licensed Practical Nurse (LPN) with a minimum of 3+ years of clinical experience with older adults
1+ years of experience with local behavioral health providers and community support organizations
1+ years of experience with MS Office
1+ years of experience in long term support services or working with older adults
Reside in the state of Indiana
Ability to travel 75% of the time within assigned territory
Access to reliable transportation & valid US driver's license
Preferred Qualifications
Experience/additional training in Motivational Interviewing, Stages of Change, Trauma-Informed Care
Experience working in team-based care
Experience in serving individuals with co-occurring disorders
Background in Managed Care
Bilingual in Spanish or other language specific to market populations
