Job Description
Join The Jewish Board as a Case Associate, where you will make a significant impact on clients' lives by developing and implementing care plans. With a strong focus on community resources and support, you'll work closely with clients to ensure their needs are met in a compassionate and effective manner.
Key Responsibilities
Function as a Case Planner to provide appropriate interventions
Develop action plans for client care
Conduct routine community contacts
Locate and utilize community resources for clients
Perform home visits to evaluate living conditions
Run groups under social worker supervision
Evaluate clients for individualized treatment programs
Provide case management and individual/group counseling
Perform community outreach and establish linkages
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual in Spanish
Bilingual in Haitian-Creole
