Job Description

Join Optum as a Senior Community Center Nurse Practitioner/Physician Assistant to provide vital care to patients in their homes and community settings. Key Responsibilities

  • Deliver cost-effective, quality care to assigned members
  • Manage medical, behavioral, chronic, and acute conditions
  • Perform comprehensive assessments and document findings
  • Facilitate agreement on the member’s plan of care
  • Evaluate effectiveness and efficiency of care plans
  • Coordinate care as members transition between different levels of care

Required Qualifications

  • Certified Nurse Practitioner through a national board or Certified Physician Assistant
  • Graduate of an accredited master's degree in Nursing or Physician Assistant program
  • Active and unrestricted license in the state of residence
  • Current active DEA licensure/prescriptive authority
  • Ability to lift a 30-pound bag and navigate various dwelling conditions
  • Access to reliable transportation

Preferred Qualifications

  • 1+ years of hands-on post grad experience within Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills including electronic medical records
  • Proven ability to develop and maintain positive customer relationships
