Job Description
Join Optum as a Senior Community Center Nurse Practitioner/Physician Assistant to provide vital care to patients in their homes and community settings. Key Responsibilities
Deliver cost-effective, quality care to assigned members
Manage medical, behavioral, chronic, and acute conditions
Perform comprehensive assessments and document findings
Facilitate agreement on the member’s plan of care
Evaluate effectiveness and efficiency of care plans
Coordinate care as members transition between different levels of care
Required Qualifications
Certified Nurse Practitioner through a national board or Certified Physician Assistant
Graduate of an accredited master's degree in Nursing or Physician Assistant program
Active and unrestricted license in the state of residence
Current active DEA licensure/prescriptive authority
Ability to lift a 30-pound bag and navigate various dwelling conditions
Access to reliable transportation
Preferred Qualifications
1+ years of hands-on post grad experience within Long Term Care
Understanding of Geriatrics and Chronic Illness
Proficient computer skills including electronic medical records
Proven ability to develop and maintain positive customer relationships
