Job Description

Join Empatia as a Palliative Care Nurse Practitioner and provide compassionate, advanced care to patients in their homes. Collaborate with a dedicated team to enhance the quality of life for those with complex medical needs.

Key Responsibilities

  • Collaborate with the Palliative Medical Director and care team to deliver comprehensive palliative care
  • Perform in-depth patient assessments
  • Develop and implement individualized care plans
  • Manage and adjust treatment plans based on patient needs
  • Prescribe medications and order diagnostic tests
  • Maintain accurate clinical documentation
  • Communicate effectively with healthcare professionals
  • Participate in evening/weekend call rotation as needed
  • Facilitate referrals for additional care services

Required Qualifications

  • Master of Science in Nursing from an accredited school
  • Current RN license and Nurse Practitioner certification
  • Minimum two years’ Nurse Practitioner experience
  • Collaborative agreement with supervising physician
  • Federal DEA license and state DEA license (if required)

Preferred Qualifications

  • Specialty in Family or Adult Medicine
  • Experience with Part B billing
  • ACHPN certification in Hospice and Palliative Care
