Shift: Days | Monday – Friday | 8:00 AM – 5:00 PM
Hours/Week: 40

Position Overview:
Join a leading Home Health & Hospice team in Littleton, NH. This role offers a unique opportunity to provide one-on-one patient care in the home setting across all stages of life. The RN will develop individualized plans of care, collaborate with a multidisciplinary team, and enjoy a high level of autonomy with strong clinical support. Responsibilities:
Deliver skilled nursing care in patient homes for both Home Health and Hospice
Develop and manage individualized plans of care
Collaborate with interdisciplinary team members
Educate patients and families on disease processes and care plans
Maintain accurate EMR documentation
Drive to patient homes within service area
Requirements:
Active NH RN License – Required
BLS – Required
2–5 years of recent Med-Surg experience – Required
Home Health experience – Preferred
Must be self-directed and able to work independently
EMR proficiency – Required
Strong clinical assessment and patient communication skills
Additional Information
No on-call or weekend requirements
Home care environment – must be flexible and adaptable
