Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of clients by providing case management and community support. Work collaboratively to develop action plans and access resources for effective client care.
Key Responsibilities
Function as a case planner and supply appropriate interventions for client care
Develop action plans utilizing existing and new resources
Conduct routine community contacts and home visits
Evaluate clients for individualized treatment programs
Perform individual/group counseling
Provide case management and community outreach
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
