Key Responsibilities
Provide and document skilled nursing care under RN supervision
Observe and assess patient condition and report changes
Document all activities and nursing actions in a timely manner
Participate in updating the nursing care plan
Educate patients and families on disease processes and self-care
Required Qualifications
Active, unencumbered LPN/LVN license in Texas
Current CPR certification
TB skin test (current within last 12 months)
Reliable transportation
Preferred Qualifications
Six months prior hands-on nursing experience
Home health experience
