Job Description
Join The Jewish Board as a Case Manager to support adults with serious mental illness in achieving independence and stability. Utilize a collaborative approach to guide residents in personalized recovery goals, while advocating for their needs and ensuring a compassionate environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Advocates for residents with service providers
Completes documentation of resident interactions and service plans
Participates in planning and execution of special events
Facilitates monthly roommate meetings and resident support groups
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
In-depth understanding of mental illness, substance use disorders, or trauma
Experience working with individuals living with serious mental illness
Knowledge of local and state health and human service systems
Preferred Qualifications
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
