Job Description
Join BoldAge PACE as a PCP Nurse Practitioner and make a difference in the lives of seniors. You'll provide vital health care in a supportive environment while enhancing the quality of life for older adults, ensuring they live with dignity and independence.
Key Responsibilities
Provide direct and indirect care to participants in the PACE Health Clinic and Day Center
Perform health histories, physical exams, and participant teaching
Order, interpret, and follow up on lab tests and results
Document care in EMR and initiate emergency measures as indicated
Coordinate participant care under the supervision of the attending physician
Act as a liaison with healthcare professionals
Document participant changes and communicate with team members
Evaluate and treat participants in various settings
Precept medical students and residents during their PACE rotation
Encourage participants to have Advanced Directives
Required Qualifications
Master’s degree in clinical nursing from accredited Nurse practitioner program
3-5 years nursing experience, with Nurse Practitioner experience preferred
RN licensure in the respective state and a Family certification from Nurse Practitioner program
1 year of experience working with a frail or elderly population preferred
Reliable transportation and valid driver's license
Medically cleared for communicable diseases and up-to-date immunizations
Pass a comprehensive criminal background check
