Job Description
Join CarePartners as a Physical Therapist in Home Health, where you will deliver patient-centered care and work closely with physicians to manage therapy for patients in their homes, making a significant impact on their recovery and wellbeing.
Key Responsibilities
Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
Provide purposeful and skilled therapeutic care at patient’s home.
Instruct patient/family/caregiver in the use of therapies, exercises, and therapeutic appliances.
Prioritize caseload and coordinate assignments/handoffs.
Required Qualifications
Licensed Physical Therapist (must be obtained within 30 days of employment start date)
Drivers License
Bachelors Degree
