Job Description
Join The Jewish Board as a Case Manager to support adults with serious mental illness in achieving independence. You'll work closely with residents, guiding them through personalized recovery goals while advocating for their needs in a compassionate environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Advocates on behalf of residents with service providers
Completes documentation of resident interactions
Participates in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
Knowledge of local and state health and human service systems
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
