Job Description
Join Sunshine State Health Plan as a Case Manager to make a real difference in the lives of our members. You will develop and coordinate care plans, ensuring quality and cost-effective healthcare while providing essential support to members and their families.
Key Responsibilities
Develop and coordinate holistic care management activities
Evaluate member needs and recommend care plans
Coordinate between members and care providers
Provide resource support for members and families
Monitor care plans and provide recommendations
Document member information and care activities
Perform home visits to assess member needs
Facilitate education on healthcare options and benefits
Required Qualifications
Bachelor's degree
2 - 4 years of related experience
Preferred Qualifications
Equivalent experience in healthcare or case management
