Job Description
Join CenterWell Home Health as an RN Clinical Manager and lead a dedicated team in providing exceptional home care services. You'll coordinate patient care, oversee clinical operations, and ensure compliance with healthcare standards, making a significant impact on patient lives.
Key Responsibilities
Coordinate and oversee direct care patient services provided by clinical personnel
Develop, plan, implement, analyze, and organize clinical operations
Conduct and delegate patient assessments and update care plans
Manage assignment of caregivers and oversee delivery of care
Monitor compliance with regulatory agencies and third-party payers
Participate in sales and marketing initiatives
Supervise clinical employees and handle employee corrective actions
Conduct staff education and evaluate performance
Analyze situations and implement Performance Improvement principles
Required Qualifications
Graduate of an accredited School of Nursing
Current state license as a Registered Nurse
Two years as a Registered Nurse with at least one year of management experience in a home care, hospice or equivalent environment
Experience with OASIS and HomeCare HomeBase (HCHB)
Valid driver’s license, auto insurance and reliable transportation
Proof of current CPR
