Job Description
Join MJHS as a Licensed Practical Nurse (LPN) where you will provide compassionate care to patients in their homes, ensuring their recovery process is supported with skilled assessments and treatment plans.
Key Responsibilities
See patients in their home for revisits based on a skilled need
Implement the plan of care and evaluate the treatment
Document the visit electronically and complete documentation within 24-48 hours
Required Qualifications
NY State LPN (Licensed Practical Nurse) License
Minimum of one year Med/surg experience in a hospital or Longterm Care facility
Preferred Qualifications
Driver License and car may be required based on job location
