Job Description
Join UnitedHealthcare as a Field Care Coordinator, where you'll play a critical role in improving the healthcare experience for individuals in Northeast Wayne County, MI. This field-based position involves developing individualized care plans and ensuring timely access to services, advocating for patients, and coordinating care across various health settings.
Key Responsibilities
Develop and implement care plan interventions
Communicate health-related information to stakeholders
Advocate for patients and families
Assess and plan individualized care strategies
Identify barriers to care and provide interventions
Initiate referrals for social service programs
Manage person-centered service/support plans
Conduct home visits and in-person visits
Document member information and care management activities
Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan
Master’s degree and current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW)
Bachelor’s degree and current, unrestricted independent licensure as a Social Worker (e.g. LLBSW, LBSW)
2+ years of experience in community health setting
1+ years of experience with local behavioral health providers
1+ years of experience with long-term care needs
1+ years of experience in electronic documentation systems and MS Office
Access to reliable transportation and ability to travel 75% of the time
Access to a designated quiet workspace in your home
Ability to travel to Southfield, MI office for quarterly meetings
Must reside within the state of Michigan
Preferred Qualifications
RN or LMSW; LCSW, LLMSW
1+ years of medical case management experience
Experience or training in Motivational Interviewing, Stages of Change, Trauma-Informed Care
Experience serving individuals with co-occurring disorders
Experience with MI Health Link (MMP)
Experience in Managed Care
Knowledge of NCQA documentation standards
