Job Description
Join HarmonyCares, a leader in home-based primary care, as a Nurse Practitioner. You will conduct comprehensive health risk assessments and deliver quality healthcare directly in patients' homes, ensuring they receive the attention and care they deserve.
Key Responsibilities
Conduct comprehensive in-home health risk assessments
Take patient history and examine, determine diagnoses
Provide written documentation of patient visit per NCQA standards
Take patient vital signs as necessary
Place case management referrals and communicate with PCP as necessary
Communicate with patients, caregivers, agency nurses, and other providers
Perform all clinical duties while observing OSHA Universal Precautions
Maintain patient confidentiality
Attend required meetings and participate in committees
Participate in professional development activities
Required Qualifications
Active/unrestricted medical license
Active CPR Certification
Board eligible or board certified in family medicine
Outstanding EHR skills
Preferred Qualifications
Geriatric training/experience
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
