The RN Care Manager provides comprehensive case management to patients enrolled in Medicare and Medicaid managed care plans, coordinating medical, social, psychological, physical, and spiritual needs. They develop, implement, and monitor individualized care plans in collaboration with primary care providers, caregivers, and healthcare teams to support patient independence. This role requires a licensed RN with experience in managed care or related nursing fields, and a focus on improving patient outcomes in a managed care setting.
