MedAdventures is seeking a skilled and compassionate Occupational Therapist (OT) to join our team at a Skilled Nursing Facility (SNF) in Cleveland, OH. Responsibilities:

  • Evaluate and treat patients with physical, cognitive, and psychosocial impairments.
  • Develop and implement individualized treatment plans.
  • Educate patients and caregivers to support therapy goals and independence.
  • Maintain accurate documentation and track patient progress.
  • Collaborate with the interdisciplinary team to provide coordinated care.

Qualifications:

  • Active Ohio OT license (or eligibility to obtain).
  • SNF experience preferred but not required.
  • Excellent communication and problem-solving skills.
  • Commitment to high-quality, patient-centered care.
