Role Overview

The Physical Therapist evaluates, screens, and rehabilitates patients with physical functioning disorders and/or wound care needs, developing individualized plans of care in accordance with regulatory and clinical practice standards.

Key responsibilities:

• Utilize standardized assessments and evidence-based practice to guide interventions.

• Administer rehabilitation procedures including manual techniques, therapeutic exercises, ambulation, modalities, supportive/assistive devices, and wound care.

• Monitor patient progress and update treatment plans in the clinical record.

• Evaluate treatment outcomes for effectiveness and modify services as needed.

• Provide consultation and education to patients, families, caregivers, and the healthcare team.

• Maintain thorough documentation of treatments and patient progress in the electronic medical record.

• Participate in care conferences, utilization review meetings, and rehabilitation meetings.

• Obtain necessary DME or supplies as needed.

• Supervise and support PT assistants, rehabilitation aides, or students.

• Ensure adherence to APTA and state-required practice acts, codes of conduct, and ethics.

Submission Requirements

• 1 year of PT experience – Required (New Grads okay if Level II experience completed)

• LTC or Rehab experience – Required

• MO State License – Required
