Job Description

Join The Jewish Board as a Case Planner, where you'll make a meaningful impact by providing preventive services to families at risk. You'll work closely with families in their communities, conducting assessments and offering essential support to ensure the safety and well-being of children. This role blends clinical skills with community engagement to help families thrive.

Key Responsibilities

  • Provide Child Welfare Preventive Services in clients' homes and communities
  • Assess and address safety and risk concerns related to children's health and well-being
  • Evaluate living conditions through home visits
  • Provide supportive casework counseling
  • Collaborate with external resources and community members
  • Complete and update Family Assessment Service Plan Reports (FASP)
  • Monitor children regarding their medical needs
  • Participate in Family Team Conferences (FTCs)
  • Carry caseload size as identified by program service model

Required Qualifications

  • BA or Masters level degree in Psychology, Social Work, Counseling or a related subject
  • Basic social work knowledge and understanding of stages of change
  • Ability to provide in-home child welfare services
  • Ability to establish rapport and therapeutic relationship with families
  • Knowledge of Microsoft Office

Preferred Qualifications

  • Understanding of the use of family therapy principles
  • Understanding of early childhood development and related theories
  • Strong assessment skills to evaluate family dynamics
  • Knowledge and experience with the Connections system
