Key Responsibilities
Collect data and conduct focused nursing assessments
Participate in the development and modification of patient-centered health care plans
Implement nursing interventions within care plans
Assist in evaluating responses to interventions
Provide safe and effective nursing care
Advocate for the best interest of the patient
Communicate and collaborate with patients and health care team members
Assist with health counseling and teaching
Delegate and assign nursing interventions
Required Qualifications
Licensed Practical Nurse (LPN) - active license in the state of practice
