The Hospice Field Admissions RN conducts initial physical and psychosocial assessments of potential hospice patients in various settings such as private homes, hospitals, and nursing facilities. They develop individualized care plans, establish goals, and admit eligible patients following National Hospice and Palliative Care Organization standards. In this field position you will perform initial physical and psychosocial assessments of potential hospice patients in order to develop individualized plans of care, goals, and make recommendations based on each patient's needs. Assessments may take place in private homes, hospitals or nursing facilities. As a Hospice Field Admission Nurse (RN) , you will admit eligible patients onto the Hospice program following the appropriate standards of practice established by the National Hospice and Palliative Care Organization (NHPCO).
