Job Description

Join The Jewish Board as a Case Planner, where you'll make a significant impact by providing preventive services to at-risk families. You'll assess family dynamics, provide counseling, and connect families with essential resources, all while working primarily within their homes. The role emphasizes collaboration and support to ensure child safety and family stability.

Key Responsibilities

  • Provide Child Welfare Preventive Services primarily within the client's home and community
  • Assess and address safety and risk concerns related to children's health and wellbeing
  • Evaluate living conditions of clients through home visits
  • Provide supportive case work counseling to children and family members
  • Collaborate with external resources in the family's ecology
  • Conduct specialized groups for children, adolescents, and parents
  • Complete weekly progress notes and enter data into required systems
  • Attend necessary trainings and meetings
  • Carry caseload size as identified by the program service model
  • Monitor children regarding their medical needs
  • Perform outreach to communities and establish linkages with other groups

Required Qualifications

  • BA or Masters level degree in Psychology, Social Work, Counseling or a related subject
  • Basic social work knowledge and understanding of stages of change
  • Ability to provide in-home child welfare services
  • Ability to establish rapport and therapeutic relationship with families

Preferred Qualifications

  • Understanding of the use of family therapy principles to provide services
  • Understanding of early childhood development and related theories and models of practice
  • Strong assessment skills to evaluate dynamics and interactions within the family
  • Knowledge and experience with the Connections system
