Join Empatia as a Palliative Care Nurse Practitioner, where you'll provide compassionate, advanced care to patients in their homes, collaborating with a dedicated team to enhance quality of life and manage complex medical needs.
Key Responsibilities
Collaborate with the Palliative Medical Director and care team
Perform in-depth patient assessments
Develop and implement individualized care plans
Manage and adjust treatment plans based on patient needs
Prescribe medications and order diagnostic tests
Maintain timely, accurate clinical documentation
Communicate effectively with healthcare professionals
Participate in evening/weekend call rotation
Facilitate referrals for additional care services
Required Qualifications
Master of Science in Nursing from an accredited school
Current RN license and Nurse Practitioner certification
Minimum two years’ Nurse Practitioner experience
Collaborative agreement with supervising physician
Federal DEA license and state DEA license (if required)
Preferred Qualifications
Specialty in Family or Adult Medicine
Experience with Part B billing
ACHPN certification in Hospice and Palliative Care
