Key Responsibilities
Assess patient conditions and develop individualized care plans
Administer medications, treatments, and wound care while monitoring patient responses
Provide patient and caregiver education on medication management and disease progression
Monitor for changes in patient status and adjust care plans as needed
Coordinate with healthcare professionals for seamless care transitions
Document patient care and interventions accurately
Ensure compliance with infection control and home care best practices
Support families and caregivers with guidance and emotional support
Maintain communication with case managers and interdisciplinary teams
Complete charting and documentation requirements
Adhere to healthcare regulations
Required Qualifications
One year of recent experience as a Registered Nurse
Minimum of six months in a Home Health setting as an RN
Proficient in OASIS documentation
Valid RN license in compliance with state regulations
BLS certification (AHA/ARC)
Preferred Qualifications
Home Health Nursing Certification (RN-BC)
Other certifications and licenses may be required
