Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate health-related information to stakeholders
  • Advocate for patients' needs and choices
  • Assess and implement care strategies
  • Identify problems/barriers to care and provide interventions
  • Initiate referrals for social service programs
  • Provide resource support for local services
  • Manage person-centered service plans
  • Conduct home visits and in-person visits
  • Document member information and compliance

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or licensure as a Social Worker (LMSW, LCSW, LLMSW)
  • 2+ years of experience in a community health setting
  • 1+ years of experience with local behavioral health providers
  • 1+ years of experience with long-term care needs
  • 1+ years of experience with electronic documentation systems
  • Access to reliable transportation and ability to travel within assigned territory
  • Access to a designated quiet workspace at home
  • Ability to travel to Southfield, MI for quarterly meetings
  • Must reside in Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Experience in Motivational Interviewing and Trauma-Informed Care
  • Experience with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
