We are seeking a dedicated and compassionate Clinic Physical Therapist to join our team at our Queens location. As a Clinic Physical Therapist, you will play a vital role in providing high-quality care to our participants, especially the geriatric population.
Job Duties: - Assessment and Treatment: Conduct comprehensive physical therapy assessments and develop individualized treatment plans for participants based on their specific needs.
- Therapeutic Interventions: Administer physical therapy treatments, exercises, and modalities to improve participants' physical function, mobility, and overall well-being.
- Education: Educate participants and their families about their condition, treatment options, and home exercise programs to facilitate their recovery.
- Documentation: Maintain accurate and up-to-date participant records in accordance with regulatory and organizational standards using electronic medical records systems like Care Compass.
- Collaboration: Collaborate with a multidisciplinary team of healthcare professionals to provide holistic care to participants, ensuring their physical and emotional well-being.
- PACE Regulations: Stay informed and compliant with PACE regulations to ensure the highest quality of care for our participants.
- Home Visits: Travel to participant's homes when necessary to deliver therapy services.
Education: - Successful completion of a Physical Therapy program from an APTA accredited university or equivalent.
Experience & Skills Required: - Minimum of one (1) year of experience as a Physical Therapist.
- Experience working with the Geriatric population is preferred.
- Ability to establish rapport with patients and staff.
- Ability to travel to participant’s homes, nursing homes, and sites/clinics.
- Knowledge of PACE (Program of All-Inclusive Care for the Elderly) regulations.
- Knowledge of Care Compass and other Electronic Medical Records (EMRs) preferred.
