Job Description
Join UnitedHealthcare as an RN Field Clinical Care Coordinator, where you'll be the primary care manager for members with complex needs. This role combines telehealth and field work, providing you with the opportunity to improve health outcomes while working flexibly in Worcester County, MA.
Key Responsibilities
Engage members face-to-face and/or telephonically for needs assessment
Develop and implement person-centered care plans
Collaborate with internal care team and community resources
Provide education and coaching for self-management of care needs
Support proactive discharge planning and care transition management
Advocate for members and families
Required Qualifications
Current and unrestricted independent licensure as a Registered Nurse
2+ years of clinical experience
Intermediate level of proficiency with MS Office, including Word, Excel and Outlook
Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI)
Reside in a location that can receive high-speed internet
Access to reliable transportation and the ability to travel within assigned territory
Valid US driver’s license
Preferred Qualifications
Bachelor’s or Master’s Degree in Nursing
Certified Care Manager (CCM)
1+ years of community case management experience
Background in Managed Care
Experience working in team-based care
Ability to utilize an Electronic Medical Record or other platforms
Ability to use online training platforms
Demonstrated ability to utilize virtual care platforms
