Key Responsibilities
Perform initial and ongoing assessments of resident physical function and mobility
Develop, implement, and revise individualized treatment plans based on assessment results
Administer physical therapy treatments and interventions in line with best practices
Educate residents, families, and staff on proper techniques and home exercise programs
Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely
Collaborate with the interdisciplinary care team, attending care conferences and assisting with discharge planning
Supervise and delegate duties to Physical Therapist Assistants (PTAs) as required by Colorado licensure laws
Ensure compliance with Medicare, Medicaid, and facility documentation and billing standards
Maintain equipment and work areas to ensure safety and cleanliness
Required Qualifications
Doctorate or Master’s degree in Physical Therapy from an accredited institution
Current Colorado Physical Therapist License in good standing
Valid CPR certification
Strong interpersonal, documentation, and organizational skills
Ability to work effectively in a team-oriented, resident-centered environment
Preferred Qualifications
Previous experience in skilled nursing, long-term care, or geriatric rehab
