Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of clients by developing action plans and providing essential resources for care. This role involves community engagement, case management, and collaboration with social workers to ensure effective client support.
Key Responsibilities
Function as a case planner and ensure client care
Develop action plans utilizing existing and new resources
Conduct routine community contacts and home visits
Evaluate clients for individualized treatment programs
Run groups under supervision of the social worker
Provide case management and counseling to clients
Perform community outreach and establish linkages
Maintain necessary documentation and progress notes
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
