Optum is seeking a Nurse Practitioner to provide in-home clinical assessments across Georgia and South Carolina. This role offers flexible scheduling and no on-call, weekends, or holidays, allowing for an excellent work-life balance while improving patient health outcomes.
Key Responsibilities
Conduct in-home assessments and evaluations
Perform evidence-based practice screenings
Identify diagnoses for care management
Communicate findings to inform members' PCPs
Identify and intervene in urgent situations
Educate members on disease processes and medication
Required Qualifications
Active NP license or ability to obtain by start date
Active ANCC or AANP national certification
Ability to complete physical requirements of the job
Access to reliable transportation for travel
Preferred Qualifications
Additional licensure in other geographic areas
1+ years of clinical experience
Experience in Gerontology, Cardiology, Internal medicine, or Endocrinology
Home Health care or home visit experience
