Job Description:

We are seeking an experienced Home Health Registered Nurse to deliver high-quality nursing care to patients in their homes across Bellaire, TX. The RN will coordinate care with the interdisciplinary team, patients, families, and referring agencies while ensuring compassionate, evidence-based, and individualized care. This position requires the ability to work independently in varied home settings and provide comprehensive patient/family education.

Essential Functions:

1. Assess home health and/or hospice patients in stable and emergent situations to determine physical, psychosocial, and environmental needs.
2. Develop, implement, document, and evaluate individualized Plans of Care (POC).
3. Incorporate psychosocial, cultural, and spiritual factors into care planning.
4. Ensure accurate, up-to-date documentation of all patient care activities.
5. Provide care consistent with infection control and OSHA guidelines.
6. Educate patients and families regarding care, medications, diet, and home safety.
7. Supervise HHAs, STNAs, or LVNs to maintain quality and continuity of services.
8. Assess and manage pain effectively; educate patients/families on pain management techniques.
9. Arrange referrals to appropriate agencies as needed.
10. Monitor compliance with payer requirements.
11. Provide on-call services when assigned.
12. Communicate effectively with patients, families, physicians, and team members.
13. Participate in interdisciplinary meetings and case conferences.
14. Perform specialized nursing tasks such as IV therapy, phlebotomy, wound care, catheter/drain management, and respiratory therapy.
15. Perform other duties as required.

Requirements:

• Active Texas RN License – Required
• Homecare Homebase software experience – Required
• OASIS documentation experience – Required
• BLS/CPR certification – Required
• Valid driver’s license, reliable transportation, and insurance – Required
• 1-2 years of RN clinical experience – Required
• 2+ years of Home Health experience – Required
• CHPN certification – Preferred

Education:

• Associate Degree in Nursing (ADN) or Nursing Diploma Program – Required
• Bachelor of Science in Nursing (BSN) – Preferred
