Job Description
The Case Manager at The Jewish Board supports adults with serious mental illness in achieving their recovery goals and living independently. This role involves direct care, advocacy, and collaboration with residents to navigate services and enhance their quality of life.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning
Evaluates and assesses independent living skills
Advocates on behalf of residents with other service providers
Completes regular and timely documentation of resident interactions
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
In-depth understanding of mental illness and substance use disorders
Experience working with individuals living with serious mental illness
Knowledge of local and state health and human service systems
Preferred Qualifications
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
