Key Responsibilities

• Provide direct patient care using the nursing process
• Perform plan of care interventions including medication administration
• Document patient care given
• Monitor and communicate patient condition to care team
• Assist patients with activities of daily living
• Collaborate across disciplines for patient care coordination
• Evaluate patient and family learning needs
• Advocate for the physical and emotional well-being of patients
• Attend staff meetings and participate in projects
