Key Responsibilities
Provide professional, comprehensive, patient-centric care management services
Conduct assessment and planning for at-risk patients
Coordinate care and manage resource utilization
Oversee discharge planning and documentation of interventions
Ensure regulatory compliance and advocate for patients
Required Qualifications
Associate's Degree - Nursing degree/diploma upon hire
Alaska Registered Nurse License upon hire
2 years - Acute care experience in a Medical Surgical or Inpatient setting
IRR or annual competency testing in Utilization Review
Preferred Qualifications
Bachelor's Degree - Nursing or higher within 3 years of hire
National Certification in area of specialty upon hire
1 year - experience in care management or utilization review in any setting or successful completion of TIPS program or Case Management Orientation Program
