Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Supports residents in medication management independence
Advocates on behalf of residents
Completes regular documentation of resident interactions
Participates in intake, assessments, and discharge conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS)
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
