Travel LTAC RN – Case Manager (Days)

Location: Phoenix, AZ
Assignment: 13 Weeks

Setting: Long-Term Acute Care (LTAC)

HumanEdge Health is seeking a skilled Travel RN Case Manager for a day shift LTAC assignment in Phoenix, Arizona. This 13-week contract offers a unique opportunity to support patients with complex needs, playing a vital role in discharge planning, care coordination, and utilization review.

Your Role Will Include:

• Performing utilization reviews, resource management, and discharge planning

• Collaborating with the interdisciplinary care team, patients, and families to coordinate safe and effective care transitions

• Documenting treatment plans and progress notes in compliance with payor and policy standards

• Assisting with referrals, external resources, and financial risk management

• Leading family/team conferences and ensuring satisfaction across patients, families, and payors

• Helping identify and eliminate unnecessary treatments, tests, or appointments

• Managing outliers and maintaining compliance with national and local contract requirements

Qualifications:

• Current RN license in Arizona (or compact license)

• 2+ years of acute care or case management experience preferred

• BLS certification required

• Familiarity with MedHost, UR tools, or commercial payor systems a plus

• Strong clinical judgment, organizational skills, and team collaboration
