Job Description
Join CenterWell Home Health as a Home Health Physical Therapist, where you'll empower patients to regain mobility and independence through personalized rehabilitation care, all from the comfort of their homes.
Key Responsibilities
Plan and administer prescribed skilled physical therapy treatment
Test/screen the patient’s physical strengths
Develop/implement a conditioning/rehabilitation program
Confer with physician and clinical team members
Provide instructions/care plans for Physical Therapy Assistants
Document patients' care observations and interventions
Report patient's progress to the physician and care team
Submit evaluation, treatment plans, and progress reports
Required Qualifications
Degree from an accredited Physical Therapy Program
Current and unrestricted Physical Therapy license
Current CPR certification
Strong organizational and communication skills
Preferred Qualifications
Minimum of one year physical therapy experience preferred
