Job Description
Join South Shore Health as a Community Outreach Liaison, where you will play a vital role in managing patient referrals for home care services. This position involves building relationships with healthcare professionals and patients, assessing needs, and ensuring smooth admissions to provide exceptional care.
Key Responsibilities
Identify and prioritize patients requesting home care services
Consult with SSH care managers regarding home care programs
Develop relationships with colleagues, physicians, and patients
Explain SSVNA services to patients and families
Coordinate admissions with Central Intake
Assess patient needs to meet admission criteria
Consult on complex patient discharges
Maintain communication with referral sources
Participate in community outreach activities
Review weekly admission and referral reports
Maintain statistical data of referrals
