Join the Youth Assertive Community Treatment (ACT) team as a Licensed Behavioral Health Clinician, providing crucial support to youth aged 10-21 with significant behavioral health needs. Engage families in individualized treatment plans while collaborating with a multi-disciplinary team to foster health and re-integration into the community.
Key Responsibilities
Provide treatment to the child and their family/caregivers
Engage child/youth and family as active partners in developing care
Assess risk, develop safety plans, and provide crisis intervention
Participate in the development and implementation of person-centered care plans
Establish collaborative working relationships with community providers
Complete casework documentation and collect data
Monitor and evaluate participant progress
Provide on-call after-hour crisis intervention services
Required Qualifications
Licensure (LMSW, LCSW, LMFT, LCAT) in a Behavioral Health or related field
Minimum of one year of experience providing direct services to children with serious emotional disturbance and their families
