Job Title: RN Case Manager – MSO Division
Location: Prescott Valley, AZ 86314
(Hybrid – Remote with on-site visits across regional practices)
Employment Type: FULL TIME/PERMANENT
Shift Details: 5 Days/Week | 8 Hours/Day | 40 Hours/Week

Job Summary

The RN Case Manager plays a key role in coordinating patient care, managing chronic disease programs, and optimizing clinical workflows across multiple independent medical practices. This hybrid role (remote + on-site) serves as the link between patients, providers, and the MSO leadership team ensuring quality care, compliance, and operational efficiency through data-driven processes and technology integration.

Roles & Responsibilities:

  • Coordinate care for high-risk and chronic disease patients (CHF, COPD, diabetes).
  • Oversee care transitions, follow-ups, and wellness visit compliance.
  • Implement and optimize in-office diagnostic programs (ANS, BrainView, vascular, cardiac).
  • Audit workflows to improve efficiency and reduce revenue leakage.
  • Collaborate with providers, MAs, and front-office teams to enhance operations.
  • Serve as the MSO’s on-site ambassador for assigned practices.
  • Report performance metrics and support practice growth initiatives.
  • Ensure compliance with CMS, HIPAA, and quality measures (HEDIS, MIPS, CCM, TCM).
  • Train and support clinic staff on SERVE MD OS protocols and best practices.

Requirements:

  • Active, unrestricted RN license (multi-state compact preferred).
  • Minimum 3 years of clinical experience (Internal Medicine, Family Medicine, or Geriatrics preferred).
  • Prior experience in case management, population health, or care coordination.
