Key Responsibilities
Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment
Provide health education and physician referrals
Collaborate with physicians, case managers, and other medical personnel
Document patient interactions according to Matrix guidelines
Provide services in various venues including home visits and skilled facility visits
Required Qualifications
Master’s Degree or commensurate experience and satisfactory completion of NP licensure
Current RN and NP licensure in state of practice with prescription authority
Board certified by AANP, ANCC, or AACCN in a Matrix approved specialty
Current BLS, ACLS, or CPR certification
Minimum of 1 year experience as a Nurse Practitioner
Strong computer skills and familiarity with employee health/medical record software
