Our member-centered approach focuses on individualized care to meet the unique needs of our members and is supported by our dedicated care team of primary care physicians, health coaches, nurses and an array of specialized services. AdventHealth Senior Care is creating a new connection around how whole-person care is delivered to better meet the full spectrum of health care needs in this senior population. AdventHealth Senior Care is looking for a full time LCSW Geriatric Social Worker to be responsible for social work practice and performing proactive outreach to our members based on their care needs. The primary responsibilities of the Geriatric Social Worker center around problem-solving, providing resources, services, and opportunities to enhance coping capacities for members. The Geriatric Social Worker serves as the subject matter expert for the member's psychosocial and behavioral health needs. The Geriatric Social Worker also contributes to the development and improvement of social policy in the primary care setting and community. The Geriatric Social Worker is an important part of AdventHealth Senior Care and supports the care team in decision-making to provide a high-quality care experience for members.

• Function as a treatment team member, interpreting social, psychological, emotional and family system problems and strengths to other members of the treatment team.
• Prioritize clinical problems, formulate treatment goals and construct treatment plan, revising as needed, based on continuous evaluation and assessment of progress.
• Care coordination responsibilities and related functions as appropriate for patient population.
• Incorporates member, physician, customer needs and concerns into decision-making.
• Demonstrate ability to provide social work services to member, family members and significant others in four areas: psychological functioning, adaptation to illness, injury, disability or life event, psychosocial assessment/evaluation, and facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge.

The expertise and experience you'll need to succeed:

Minimum qualifications:

• Master's in Social Work
• 1 year of Position-Related Experience
• 1 year of direct clinical with case management experience preferable in a health or mental health setting with an emphasis on mental health, substance abuse and/or geriatric population
• License Clinical Social Worker Maintains Florida State (Chapter 491 Florida Statues)
