Key Responsibilities
Provide basic nursing care and assist in resident assessment
Communicate significant changes in resident condition to the interdisciplinary team
Document assessments and interventions accurately
Participate in the care planning process and update care plans as necessary
Administer medications and treatments safely and timely
Use critical thinking skills for resident care
Notify physicians of changes using SBARR communication
Monitor care delivery and identify problem areas
Assist with residents' activities of daily living (ADLs)
Required Qualifications
Education from an accredited school of nursing resulting in LPN certification
Current LPN license in the state of employment
CPR and AED certification (or willingness to obtain)
IV certification (or willingness to obtain)
Ability to assess basic nursing care needs of geriatric residents
Excellent communication skills
Time management and organizational skills
Ability to stand and walk for long periods
Ability to lift heavy objects using proper techniques
Ability to transfer and move patients weighing 200+ lbs
Ability to push and pull equipment weighing in excess of 40 lbs
Manual dexterity and coordination
Visual acuity to distinguish color
Ability to perform documentation electronically and in writing
