Join The Jewish Board as a Case Planner, where you'll make a meaningful impact by providing preventive services to at-risk families. Work collaboratively in home and community settings to ensure children's safety and support family well-being through counseling, advocacy, and access to vital resources.
Key Responsibilities
Provide Child Welfare Preventive Services in clients' homes and communities
Assess and address safety and risk concerns related to children's health and well-being
Evaluate living conditions through home visits
Provide supportive case work counseling to children and families
Identify and access concrete services and community resources
Collaborate with external resources in the family’s ecology
Conduct specialized groups for children, adolescents, and parents
Complete weekly progress notes and data entry
Attend conferences and community-based meetings
Maintain case records consistent with agency standards
Required Qualifications
BA or Masters level degree in Psychology, Social Work, Counseling or a related subject
Basic social work knowledge and understanding of stages of change
Ability to provide in-home child welfare services
Ability to establish rapport and therapeutic relationship with families
Knowledge of Microsoft Office
Preferred Qualifications
Understanding of the use of family therapy principles to provide services
Understanding of early childhood development and related theories and models of practice
Strong assessment skills to evaluate dynamics and interactions within the family
Knowledge and experience with the Connections system
