Job Description
The Case Manager at The Jewish Board plays a crucial role in supporting adults with serious mental illness, helping them achieve independence in a community setting. This position requires compassionate advocacy and collaboration to develop personalized recovery goals.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Advocates on behalf of residents with other service providers
Completes regular documentation of resident interactions
Participates in planning special events and regular staff meetings
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
