Job Description
Join Optum as a Nurse Practitioner or Physician Assistant in Senior Care Patient Connect, where you'll provide crucial support to patients transitioning from rehab to home. This role allows you to make a significant impact in patients' lives by ensuring they receive the care they need while working independently in diverse settings.
Key Responsibilities
Perform comprehensive assessments and document findings
Ensure all diagnoses are ICD10 coded accurately
Address and document all quality elements
Conduct initial medication review
Complete mandatory educational and training requirements
Travel between care sites as required
Communicate with the patient and team members regarding medical concerns
Required Qualifications
Certified Nurse Practitioner through a national board
Active and unrestricted APC license in the State of Connecticut
Current active DEA licensure/prescriptive authority or ability to obtain post-hire
Driver’s license and access to reliable transportation
Availability to work 10 hours per week
Ability to gain a collaborative practice agreement
Preferred Qualifications
1+ years of hands-on post grad experience within Long Term Care
Ability to move a 30-pound bag and navigate stairs
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Proficient computer skills including medical documentation
Ability to develop and maintain positive customer relationships
