As a Care Manager, you will play a vital role in transforming health outcomes for our members by developing personalized care plans and coordinating services for those with mental and behavioral health needs. Key Responsibilities
Develop and facilitate complex care management activities
Conduct evaluations via phone or in-home visits
Develop ongoing care plans for high acuity members
Coordinate care between members, families, and providers
Monitor care plans and member status
Educate members and families on healthcare options and benefits
Document care management activities for compliance
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2 – 4 years of related experience
Licensed Master's Behavioral Health Professional (LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Bilingual candidates highly preferred
Experience with mental health and substance use disorders
Professional pediatric care experience for Florida's Children’s Medical Services
