Job Description
Join the Youth Assertive Community Treatment (ACT) Team as a Licensed Behavioral Health Clinician to make a meaningful impact on the lives of youth aged 10 to 21 facing significant behavioral health challenges. You will work collaboratively in a multi-disciplinary environment, providing individualized treatment and support to families, helping them navigate their journey towards improved health and stability.
Key Responsibilities
Provide treatment to children and their families to address clinical and complex needs
Engage families in developing and modifying care plans
Assess risk and provide crisis intervention services
Participate in person-centered care plan development and implementation
Monitor progress and provide ongoing care planning
Collaborate with community providers and other service systems
Complete casework documentation and report data
Provide on-call crisis intervention services when needed
Required Qualifications
Licensure (LMSW, LCSW, LMFT, LCAT) in a Behavioral Health or related field
Minimum of one year of experience providing direct services to children with serious emotional disturbance and their families
