Job Description
Join Monte Nido & Affiliates as a Social Worker, where you'll play a vital role in care coordination and case management for clients with eating disorders. This hybrid position involves supporting both adults and adolescents across multiple treatment programs, ensuring seamless access to community resources and facilitating transitions in care.
Key Responsibilities
Care coordination and case management for clients and families
Assist in connecting clients and families with community-based programs and resources
Send referrals to community programs and facilitate collaboration with outpatient providers
Help facilitate smooth transitions between various levels of care
Provide emotional support and necessary information to clients and families
Ensure clients receive continued care post-transition
Required Qualifications
Master’s degree in Social Work or related field
Experience with CareOregon/Medicaid
Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
Strong communication skills
Ability to work collaboratively with a multidisciplinary team
Strong understanding of available community resources, government programs, and insurance processes
Preferred Qualifications
LCSW preferred
